Walk in the right way martial arts seminar and training event held on the 14 October 2006 (the “Event”) and organised by the Dave Lee Fund a company limited by guarantee, Company No 5830048, registered office 40 Kingsmere Close, Erdington, Birmingham West Midlands B24 8QL (the “Fund”). |

The submission of the application form for places at the Event is your binding agreement to be bound by the terms herein whether your application is to participate in the event or to be a spectator. You must sign this form yourself and if you are under 18 years old your parent or legal guardian must sign it also at the appropriate place and he or she shall be responsible for the actions and attitude of the minor and shall ensure that they are within the confines of the training room at all times the minor is training. |

An application form must be completed and sent to the Fund with the appropriate fee for each person who wishes to attend the event, whether they intend to be a participator or a spectator at the Event. |

The closing date for applications to reach the Fund is 22 nd September 2006 . Thereafter no applications will be accepted. Proof of posting is not proof of delivery and the Fund, its officers or agents cannot be held responsible for the loss or not receipt of applications or any losses arising thereof whatsoever howsoever caused. |

All fees must be paid in cleared funds by the 30 September 2006 . No training fee is transferable No refunds will be made save at the sole discretion of the Fund. |
Instructors |

All the instructors featured in the advertising of the Event including featured on this website agreed to teach at the Event at the time this website was originally constructed. However each instructor is donating their services to the Event and therefore neither the Fund nor its officers nor its agents can be responsible for any loss or disappointment should any or all of the featured instructors withdraw from the Event. Please note the purpose of the event is to raise money to buy two wheelchairs. |

Numbers of places at the Event are strictly limited, after all the available places have been allocated, cheques and application forms received thereafter will be cancelled and destroyed. Prior to all places being allocated cheques may be cashed. Tickets will be sent to those who have been allocated places on or before 30 th September 2006. Fees for unsuccessful applications may be returned after the event. If you have not received tickets, or a refund or an email notification that you were unsuccessful by 6 October 2006 you must email the Fund via this website or telephone 01283 763768 on or before Monday 9 th October 2006 leaving an evening contact telephone number. The Fund cannot be responsible for you not receiving your tickets in time if you fail to make contact with it by the 9 th October 2006 and no refund will made for tickets not received by you. |

The allocation of places at the Event is at the sole discretion of the Fund and no correspondence can be entered into in respect of same. |
At the Event |

You must produce a valid licence from a recognised martial arts organisation when you register at the Event. No licence, no training, no refund, no exceptions. |

You will be allocated a training area or mat (“Mat”) within the training hall. You must only train on that Mat allocated to you. The instructors will come to the Mat. We recognise that different students will have different preference and in order for you to obtain the most appropriate training for your needs the Mats are divided into two groups (“Rotations”) you must select the appropriate Rotation on the booking form. |

Training may only take place in designated areas at designated times. |

The Fund, its officers or agents have the right to expel any participant or spectator for any breach of these terms by themselves or anyone in there care or charge. Any unreasonable and / or violent behaviour will result in exclusion from the event. In the unfortunate event of such an action being taken no refund of all or any part of any fee paid will be made. |

You understand that the safety of all participants is paramount and you will obey immediately any instruction to stop training, or to leave the training area or to leave the building. |

Any person believed (in sole discretion of the Fund, its officers or agents) to be taking or have taken illegal substance or have taken an excessive level of alcohol shall be expelled from the event and may be reported to the police. In any event, no alcohol is to be brought into the designated dojo (training area) at any time. |

The Fund acting by its agents can and may refuse admission to any person to the Event without giving a reason for the refusal. |

You are responsible for the safety and security of all your belongings and valuables at all times and no claim whatsoever howsoever arising may be brought against the Fund its officers or its agents or the instructors in respect thereof. |

Should you have any concerns about any aspect of the Event before the 14 th October 2006 you should raise your concern with the Fund at the earliest opportunity and prior to attending the Event. Once the event has begun, you should raise any concerns with one of the stewards at the Event. |

You will address all masters and teachers by their title at all times. |

You will show due respect to all senior grades at all times and you will obey any reasonable instructions issued by senior grades or relevant officers, the Event stewards, masters and teachers immediately. |

Everyone must bow when entering or leaving any designated dojo (training) area. |

12 |
All Keigogi (training clothing) should be clean and in a good state of repair. |
Photography |

You hereby agree to allow the Fund or any person authorised by the Fund, its officers or agents to photograph and /or film/ video and /or sound record any aspect of the Event including any aspect of your participation or attendance at the Event. You also hereby agree that any and all rights in respect of any and all of the images/recordings so made lie solely with the photographer, video / film or sound recorder. |
Medical conditions or disabilities |

Any existing medical condition, disability or other matter or condition which may affect you whilst at the Event must be notified to the Fund in writing and sent with the completed application form (the “application form”). Any medical condition, disability or other matter or condition which may affect you whilst at the Event that arises after the booking form was completed but before the 30 September 2006 must be notified to the Fund on or prior to the 30 September 2006 . In the event that you develop any medical condition, disability or other matter or condition which may affect you whilst at the Event on or after the 30 th September 2006 you must not attend the Event. In such circumstances any refund shall be at the sole discretion of the Fund. |

Please remember at all times the purpose of the event is to raise money to purchase Dave Lee two wheelchairs. Everyone helping with the event is giving their time free and all wish you to have a truly great days training. It is hoped that it will prove a memorable seminar for each and every student. |
